Services may be covered in full, or in part by your health insurance. If we are in-network, we will automatically bill the insurance company for you, however our agreement is between you and us, and not your third party payers. This means that you are responsible for the bill, should the insurance company deny payment.
In the rare chance that we are not contracted with your insurance company, we can provide you with the procedure codes in advance, so you can find out what portion of our services may be covered in your out-of-network policy. We will give you an itemized statement of services after each appointment that will include appropriate procedure, diagnosis, and billing codes.
Payment is expected at the time of service. Once you have paid Florida Center for Brain and Mind, we will also provide you with a copy of the paid invoice. You may then submit this invoice to your insurance company for reimbursement.
Copays are due at the time of service.
For your convenience, you may check out via encrypted and secure Paypal. We can also send you an invoice to be paid via paypal.
Cash and check and all major credit cards are accepted for payment.
Since we reserve a significant time for you that does not allow other patients to make use of that time, we would like to ask you to make every possible effort to keep your scheduled appointment.
If you need to cancel an appointment, please provide at least 48 hours notice so that we can reschedule the appointment time.
Should a psychotherapy appointment be canceled with less than 24 hours notice, unless it is an emergency, you will be required to pay a cancellation fee prior to the next scheduled appointment. Should a neuropsychological evaluation appointment be cancelled with less than 48 hours notice, you will be charged for a portion of the entire time, to be paid prior to the next scheduled appointment.
